Reduction in Amount of Insurance: The total amount of life insurance reduces by 50% on the premium due date on or after your 65th birthday, with an appropriate adjustment in premium.
Accelerated Benefit for Terminal Illness: This benefit allows you (and your spouse if covered) a one time option to receive up to 50% of the group term life insurance proceeds, upon being diagnosed by a physician as having twelve months or less to live as long as you are under age 70. Double Benefit for Accidental Death: If you die as a result of a covered injury within 365 days after the date of the accident that caused the injury; an additional benefit will be paid. The Accidental Death Insurance Benefit will be paid in accordance with your beneficiary designation. Certificate.
Waiver of Premium: If you become totally disabled, as defined in the certificate, before age 60, and the disability continues for more than 9 consecutive months, you won’t have to pay your premiums for as long as the disability lasts and benefits are payable. Coverage will terminate per the terms of the certificate of insurance.
ELIGIBILITY & EFFECTIVE DATE: All association members up to age 65 who are U.S. citizens and reside in the United States are eligible for this life insurance coverage, as well as:
An Eligible Person's spouse who: is under age 65; is a U.S. citizen and has permanent residence in the United States of America; and is not legally separated or divorced from the Eligible Person.
An Eligible Person's unmarried child, stepchild, legally adopted child or foster child who is primarily dependent on the Eligible Person for support and maintenance and who is:
- at least 15 days old but under age 19; or
- age 19 but under age 25 and is in full‑time attendance at an institution of learning and financially dependent upon the Eligible Person for support.
Effective Date: You will become covered under the policy on the certificate effective date shown on the Schedule subject to payment of the required premium.
TERMINATION: Coverage terminates on the earliest to occur of 1) the date the policy is cancelled; 2) the premium due date on or next following the date; a) you are no longer an active member of the policyholder; b) you reach age 70; or c) you fail to pay any required premium; 3) the first day of the month on or next following the date you request to cancel your coverage under the policy; or 4) the date the death benefit is paid.
Coverage of a Covered Dependent terminates on the earliest to occur of:
- the date the policy is cancelled; or
- the Premium Due Date on or next following the date he or she:
- in the case of a Covered Spouse, is legally separated or divorced from you;
- in the case of a Covered Child, no longer qualifies as an Eligible Child as defined;
attains the policy Age Limit shown in the Schedule; or
- with respect to spouses:
- the date the spouse is eligible as an insured member; or
- the date of interlocutory decree of divorce, or the date of the final decree of divorce; or
- the Premium Due Date the required premium is not made, subject to the Grace Period provision; or
- the date your coverage terminates; or
- the date of termination of Dependent's insurance under the certificate; or
- the date your Dependent enters the armed forces of any country. Membership in the reserves or a call to active duty for 2 months or less is not deemed entry into the armed forces; or
- the date the Death Benefit is paid.
However, if dependent's coverage would terminate because of your death, coverage will continue until the Premium Due Date on or next following your death.
ACCELERATED BENEFIT FOR TERMINAL ILLNESS

SATISFACTION GUARANTEED: You have a 30 day right to examine the certificate of Insurance. The certificate may be returned within 30 days of the effective date, provided no claims have been submitted or paid, and all premiums will be refunded.
