To initiate a claim for the PAGE sponsored Group Term Life  policy 68023-1-3, please call the PAGE membership department at 800-334-6861 and let them know of the insured person's death. PAGE will certify the membership status and notify NEBCO (PAGE's Third Party Administrator) to send a claim form. Print these instructions and place with your valuable papers.
