To initiate a claim for the PET sponsored $10,000 AD&D policy ADD11038, please call the PET office at 800-471-4867 and let them know of the member's death. PET will certify the membership of the deceased and notify NEBCO (PET's Third Party Administrator) to send you a claim form. Print these instructions and place with your valuable papers.
