The answer may be Accidental Death and Dismemberment (AD&D) insurance. When income is lost because of covered injury or death, AD&D provides a monetary bridge for families to help get them through the troubled times. It can prevent a major decline in your income and lifestyle while a key family member recovers and it can help provide a source of funds to prepare the injured person for a new career, if that should become necessary.

You and your dependents are covered 24 hours a day, anywhere in the world. You're covered whether you're at home, at work or on vacation. And these benefits are payable in addition to any other insurance you have (Workers’ Compensation, Unemployment Compensation or Social Security).

PAGE automatically provides its members $10,000 of AD&D coverage.  Additionally you can elect optional AD&D coverage as shown below.

*Coverage reduces to 50% on the premium due date on or after your 65th birthday and terminates on your premium due date on or after your 70th birthday.  All eligible children have $10,000 of coverage.  Other coverage amounts are available by calling the administrator. You cannot be singled out for a rate increase, rates can be increased on a class basis only.  Premiums are billed quarterly.  Rates shown are guaranteed to 7/1/2018.

ACCIDENTAL DEATH AND DISMEMBERMENT BENEFIT:  If any of the following losses result from an injury sustained in a covered accident, and the loss occurs within 365 days after that accident, this plan will pay:

For Loss of:

  • Life ................................................................100% of Benefit Amount
  • Both Hands or Both Feet or Sight of Both Eyes..... 100% of Benefit Amount
  • One Hand and One Foot ....................................100% of Benefit Amount
  • Loss of speach and hearing in both ears................100% of Benefit Amount
  • One Hand or One Foot and Sight of One Eye ........100% of Benefit Amount
  • One Hand or One Foot or Sight of One Eye ........... 50% of Benefit Amount
  • Speech ……………............................................ 50% of Benefit Amount
  • Hearing in Both Ears..............................................50% of Benefit Amount
  • Thumb and Index finger of same hand...............25% of Benefit Amount

              The amount of coverage you and your are insured for will be shown in your Schedule of Benefits.

DOUBLE BENEFITS for COMMON CARRIER:  If your injury results in loss of life within 365 days after the date of a covered accident, the plan will pay the benefit amount. If the loss occurs while riding as a passenger on any common carrier, the plan will double the benefit amount to a maximum of $10,000 for basic ADD and $300,000 for optional ADD.

EDUCATION BENEFIT:  f your children are covered under this plan and a benefit is payable because of your covered death, the plan will pay an Education Benefit to each of your eligible dependent children who qualifies. The Education Benefit will be 5% of your benefit amount, payable once per year for up to four years to a maximum of $3,000 per year.

SATISFACTION GUARANTEED:  You have a 30 day right to examine the certificate of insurance. If you are not satisfied for any reason, and no claims have been paid or submitted, you may return the certificate within 30 days of the effective date and all premiums will be refunded.

Eligibility and Effective Date:  All members that are at least 18 years of age and under age 70 who are actively performing the normal duties of their occupation and who are full-time residents of the United States are eligible for this coverage. Spouses under age 70 who are full-time residents of the United States and not legally separated or divorced from you as well as unmarried dependent children less than 24 years of age who are primarily dependent on you for support are also eligible. Your coverage ends on the premium due date on or after your 70th birthday.  Your effective date will be the first date of the month on or next following the date we receive your enrollment form and your premium payment.

In no event, will the spouse benefit amount exceed your in force benefit amount.

Termination of Insurance:  Your insurance stops on the earliest of the following dates:

  • The last day of the month during which you are no longer eligible for insurance under the Group Policy.
  • The date the Group Policy stops.
  • The end of the period for which you paid premiums, if you do not make the next required premium contribution when due.
  • The premium due date after your 70th birthday.

ReliaStar Life stops providing a specific benefit to you on the date that benefit is no longer provided under the Group Policy.

Accidental Death and Dismemberment Exclusions

ReliaStar Life does not pay benefits for loss directly or indirectly caused by any of the following:

  • An accident occurring before the effective date of the Group Policy.
  • Suicide or intentionally self-inflicted injury, while sane or insane.
  • Physical or mental illness.
  • Bacterial infection or intentional bacterial poisoning. Exception: Infection from a cut or wound caused by an accident.
  • Riding in or descending from an aircraft as a pilot or crew member.
  • Any armed conflict, whether declared as war or not, involving any country or government.
  • An accident which occurs while in the military service for any country or government.
  • An accident which occurs when you commit or attempt to commit a crime.
  • Use of any drug, narcotic or hallucinogenic agent, unless prescribed by a doctor or taken as directed by a doctor or the manufacturer.
  • Your intoxication. Intoxication means your blood alcohol content meets or exceeds the legal presumption of intoxication under the laws of the state where the accident occurred.
