To initiate a claim for the PAGE sponsored optional coverage, AD&D policy ADD11037, please call the PAGE membership department at 800-334-6801 and let them know of the member's death. PAGE will certify the membership of the deceased and notify NEBCO (PAGE's Third Party Administrator) to send you a claim form.
